Canadian Men’s Health Foundation (“CMHF”) has developed its web services, including the menshealthfoundation.ca website, the dontchangemuch.ca website, the canadianmenshealthmonth.ca website, the YouCheck tool at youcheck.ca and the eShe tool at eshe.ca (each a “Web Service”, collectively the “Web Services”), as electronic resources to educate and build awareness about men’s health issues.

We are committed to providing users of our Web Services with services that respect their privacy. This page summarizes the privacy practices at our Web Services. It outlines the information we collect through our Web Services and how we use that information.

Before using our Web Services or submitting your personal information to us, please read this Privacy Policy carefully to learn about our privacy practices. By using our Web Services you signify your consent to the collection, use and disclosure of your personal information in accordance with this Policy.

Users of the Web Services should also review the Terms of Use applicable to our Web Services.

THE INFORMATION WE COLLECT AND HOW WE USE IT

INFORMATION YOU PROVIDE TO US

We collect personal information which you supply to us voluntarily when you use our Web Services. This includes information that you provide when you:

  • register for services offered in our Web Services,
  • complete surveys,
  • subscribe for newsletters and up-dates,
  • complete your subscriber profile,
  • contact us through our Web Services,
  • enter a contest.

Personal information voluntarily provided by you via our Web Services will be used for the purpose for which it was provided (for example, to take a survey, enter a contest, participate in an online forum, send an inquiry, or send an email address to subscribe for newsletters and up-dates) or to which you otherwise consent or as otherwise required or permitted by law. Such information may also be used for internal analysis for management purposes. Health survey information may also be used for analysis and research purposes, as more fully described below.

Please be aware that when you voluntarily disclose information in a forum area or other public communication service of our Web Services, your personal and other information disclosed in your communication will become public information and can be collected and used by outside parties. Such activities are beyond the control of CMHF. You should exercise caution when disclosing personal information in these areas.

YOUCHECK AND ESHE SURVEY TOOLS

We collect personal information about you when you complete the YouCheck or the eShe survey tools (the “Tools”). Your survey responses are stored on secure servers located in Canada. This information is used by the Tools to provide you with information about your health risk factors and suggestions for healthy life style improvement.

The Tools were developed by the University of British Columbia and CMHF, and information collected by CMHF through the Tools is used for analysis and research purposes related to men’s and women’s health. All published research findings will be anonymous.

You are not required to register with us in order to use the Tools, however registration permits the Tools to provide a more personalized health risk assessment when surveys are taken over time. It also enables you to track your survey results and your health risk trend over time. If you choose not to register you may use either of the Tools anonymously.

If you register in either of the Tools we ask for your first name and email address and you are required to select a password. Please be aware that if you register to use the Tools using your Facebook or Twitter account we will collect any of the following information that you have made available in your public profile in your Facebook or Twitter account:

  • email address
  • name
  • age
  • language
  • gender
  • birthday

If you choose to register, we may use the email address you provide to contact you periodically to provide you with notices relating to the Tools.

With your consent we will also use your email address provided during registration to contact you about opportunities to participate in further research, including research applicable to your particular health risk profile as disclosed by your survey results, and to provide you with information and up-dates regarding our Web Services and men’s or women’s health issues. If you no longer wish to be contacted by CMHF for these purposes you may so notify us by contacting us at [email protected].

SUBSCRIPTION SERVICES

Our Web Services provide you with the opportunity to subscribe to our Newsletter and Updates service. When you subscribe we use your email address to send you newsletters and other information and updates regarding our Web Services, men’s health issues and opportunities to support us and our work.

When you subscribe we use the information to create a Subscriber Profile for you. You may subscribe by providing your email address. If you subscribe using your Facebook or Twitter account we collect any of the following information that you have made available in your public profile in your Facebook or Twitter account:

  • email address
  • name
  • age
  • language
  • gender
  • birthday

Your Profile information is used for internal analysis purposes to better understand the demographics of our users and to improve our services. This information may also be used to personalize messages and health information provided through our subscription services.

You may unsubscribe from our Newsletters and Updates service at any time by using the unsubscribe functionality in our Newsletters and Updates emails.

INFORMATION COLLECTED WHEN YOU CONTACT US

Our Web Services allow you to contact us by email or through Contact functionality in the Web Services. We use your contact and other information provided for the purpose for which it was provided, for example, to respond to your requests for information and services. We use your feedback to assist us in evaluating our programs and to improve our Web Services.

INFORMATION AUTOMATICALLY COLLECTED

We maintain server logs and web logs which automatically collect certain information regarding users of our Web Services, such as the Internet Protocol (IP) address of your computer, the IP address of your Internet Service Provider, the date and time you access our Web Services, the length of your visit, the pages you look at, details of how you used our service (such as your search queries), the websites you visit before and after leaving our Web Services, and cookies that may uniquely identify your browser. This information may be used for internal analysis for management and website administration purposes, so that we can better understand usage of our Web Services and to improve our users’ experience. This information may be used and archived for future use in order to facilitate your online experience and to gather broad demographic information.

Our Web Services may use “cookies”, a technology that installs information on a user’s computer to track how our visitors use the Web Services and to permit the Web Services to recognize future visits using that computer. Cookies may be used to provide a more personalized experience at our Web Services. You may choose to decline cookies, if your browser permits, by modifying the Preferences section of your Internet browser software, but doing so may affect your use of our Web Services and your ability to access certain features of our Web Services.

We may use services hosted by third parties such as Google Analytics to automatically collect certain information regarding users of the Web Services, such as the Internet Protocol (IP) address of your computer, the IP address of your Internet Service Provider, and other data about how users like you use the Web Services, such as:

  • language of use;
  • general location information;
  • device type and screen size;
  • session information, including length of session, whether a user is a repeat user, days between uses for repeat users;
  • what pages are viewed or used.

These third party services use various technologies, including “cookies” and other anonymous identifiers,
to collect this information about you when you use the Web Services.

This automatically collected information is used for analysis for management and website administration purposes, so that we can better understand the community we serve, improve our Web Services and tailor our Web Services to our audience.

We make no attempt to link this information with the identity of individuals visiting our Web Services. We may, however, review server logs for security purposes, for example to detect intrusions into our network. The possibility exists that server log data, which contains visitors’ IP addresses, could in instances of criminal or other malfeasance, or where necessary to protect the security and integrity of our systems, be used to trace and identify individuals and be shared with appropriate investigative bodies and others as permitted by law.

DISCLOSURE OF YOUR PERSONAL INFORMATION

As a general rule the personal information we collect from those who use our Web Services is held in strict confidence and, except in limited circumstances as permitted or required by law, is not disclosed to third parties without your consent, express or implied.

We may however share your personal information with third parties who we have engaged to provide services or functions on our behalf. We may also authorize such outside suppliers or contractors to collect personal information on our behalf as necessary to provide the Web Services. Such suppliers or contractors have access to and may collect personal information only as needed to perform their functions and are not permitted to share or use the information for any other purposes.

We may provide third parties with aggregate statistics about our visitors and use of our Web Services, including health survey response information, for research and analysis purposes. We may also provide detailed user records to medical researchers for purposes of their own research in the field of men’s health, however we will only do so in anonymized form by removal of personal identifiers such as email addresses, unless we have obtained your consent to this disclosure.

We may also disclose personal information to fulfill the purposes for which it is provided, to meet regulatory requirements, or in connection with the transfer of our undertaking to another registered charity carrying on work of a similar nature on winding-up or dissolution of CMHF, or otherwise without your consent as required or permitted by law.

THIRD PARTY SITES AND SERVICES

Our Web Services contain links to other sites. Please be aware that if you choose to leave our site via a link, we assume no responsibility for the content or the privacy practices of other websites or services. We encourage you to examine each site’s privacy policy and make your own decisions regarding the accuracy, reliability and correctness of material and information found.

ON-LINE DONATIONS

CMHF is a registered Canadian charity. Donations may be made to CMHF online through the online services of Chimp: Charitable Impact Foundation (Canada) (“Chimp”), an independent registered charity. Our Web Services provide a link to Chimp’s online service for your convenience in making a donation through Chimp to CMHF. Please note, however, that Chimp’s services are a third party service over which we have no control. We are not responsible for the privacy practices at their site, and we encourage you to review their privacy policy and the terms of use applicable to their site.

SECURITY

CMHF uses a variety of security measures to maintain the safety of your personal information in our possession or control. All user contact information and other personal information you provide through use of the Web Services is contained behind secured networks and is accessible only by a limited number of trusted employees and/or contractors who have special access rights to production system. This information is secured on servers in a controlled, secured environment, protected from unauthorized access, use or disclosure. Passwords are encrypted in CMHF’s secure database, which is also located behind secured networks.

HOW CAN I CORRECT ERRORS IN MY INFORMATION?

You can correct errors and keep your registration and subscription information current by changing and modifying information previously provided as follows:

  1. You can up-date your YouCheck Tool registration information at the YouCheck profile page.
  2. You can up-date your eShe Tool registration information at the eShe profile page.
  3. You can up-date your newsletter subscription profile information by clicking on the Update Profile link in the subscription emails we send you.

CHANGES TO THIS PRIVACY POLICY

This Policy is in effect as of March 22, 2017. The development of our procedures for protection of personal information is an ongoing process. Due to changes in technology and legal requirements and other reasons, we will from time to time review and revise this Policy. New versions of this Policy will be available by a link from the Web Services.
